Who can become a patient?
If you’re an Iowa resident age 18 or older who suffers from one of the qualifying medical conditions for medicinal cannabidiol treatment, such as cancer or untreatable pain, you may be eligible to become a patient.
Follow these steps to obtain a registration card and begin receiving treatment:
Download the application.
Download the patient application for a medical cannabidiol registration card from the Iowa Department of Public Health website.
Get certification from your physician.
Schedule an appointment with your physician to discuss using medicinal cannabidiol as a treatment option. Ask him/her to certify you suffer from one of the qualifying medical conditions for medicinal cannabidiol treatment by filling out the required physician sections on the application.
Submit the application with documentation and payment.
Hand-deliver or mail the completed application, all required documentation and the $100 annual fee* (cash or check) to the Iowa Department of Public Health at:

Approved patients will receive mailed instructions for obtaining a registration card that must be acquired from the Iowa Department of Public Transportation. For additional information about the application process, refer to the FAQ page.
* The annual fee is reduced to $25 for patients who qualify. For more information about qualification requirements, visit the IDPH website.
Visit a dispensary.
As of Dec. 1, 2018, medicinal cannabidiol will be available at five dispensaries located across the state of Iowa. MedPharm Iowa welcomes you to visit our dispensaries in Windsor Heights and Sioux City. To check in at the door, you will need your medical cannabidiol registration card and current government-issued identification.
