Who can become a patient?
If you’re an Iowan of any age who suffers from one of the qualifying medical conditions for medicinal cannabis treatment, such as cancer or chronic pain, you may be eligible to become a patient.
Follow these steps to obtain aN Iowa medical cannabis card and begin receiving treatment:
Step 1
Get certification from your healthcare practitioner.
Schedule an appointment with your healthcare practitioner to discuss using medicinal cannabis. Ask for certification that you suffer from one of the qualifying medical conditions for medicinal cannabis treatment by filling out the application.
Your application can be signed by any MD, DO, ARNP, PA, APRN or podiatrist in Iowa. Your THC purchase limit can be increased by asking your healthcare practitioner to sign the waiver form linked here.
Step 2
APPLY ONLINE.
Complete the online application for a medical cannabis card from the Iowa Department of Public Health website. A printed form is also available.
Applications may be approved as fast as 24 hours. You are eligible to purchase as soon as you are approved.

Step 3
Visit a Dispensary
Medical cannabis is available at multiple licensed dispensaries across Iowa. To check in at the door, you will need your Iowa medical cannabis card and current government-issued ID.
MedPharm Iowa welcomes you to visit our dispensaries in Windsor Heights and Sioux City. Please call ahead to schedule a consultation for your first visit to a dispensary. Be sure to complete the patient intake form before arriving.
